Programs

Clinically integrated provider network
A network of trusted health care providers, including UT Southwestern Medical Center, Texas Health Resources and independent primary care doctors and specialists, working together to deliver connected care across North Texas.
ACO REACH value-based care
Value-based, population-health care for Medicare and Medicare Advantage patients through its Accountable Care Organization model.
Provider support and network administration
Tools, services and subject matter experts for providers, including centralized administration, payor arrangements, reporting and analytics, and population health insights.
Population health management and care coordination
Use of technology, predictive analytics, machine learning and evidence-based care models to identify care gaps, support high-risk patients and improve care coordination.
Patient access to care
Access to primary care physicians and specialists throughout North Texas, with coordinated services across routine, preventive and specialized care.
Post-acute care network
Access to home health, hospice, skilled nursing facilities, dialysis providers, inpatient rehabilitation facilities and long-term acute hospitals.
Patient health resources
Resources and education on breast cancer, diabetes, cervical cancer, hypertension, colorectal cancer, medication management, falls prevention and annual wellness visits.

Southwestern Health Resources furthers the missions of UT Southwestern Medical Center and Texas Health Resources by creating a clinically integrated network of physicians, hospitals and other care providers to deliver healthcare across the continuum.

Our story
Southwestern Health Resources was created by UT Southwestern Medical Center and Texas Health Resources to bring academic medicine, community health care, and affiliated providers into one coordinated network. It grew into a clinically integrated network serving North Texas with a focus on connected care, access, and value-based care.
