Follow-up and aftercare support at 30, 60, 90, 180, and 360 days to support treatment compliance.
Home Health Care - Mobile/Traveling Social Worker
Psychosocial evaluation, community service coordination, support, crisis intervention, and liaison services for identified patients.
Basic Navigation (EBT, SNAP, TANF, Medicaid) Services
Navigation support connecting clients and family members with social, health, and community services.
Returning Citizens ReEntry Program

Participants may self-refer or be referred by family members, courts, the Nevada Department of Children and Families, Adult Probation, Juvenile Diversion Programs, and others. The organization works on a referral system, accepts most Medicaid and commercial insurance plans, and does not accept cash, though incremental payments are accepted.
